The Department of Labor has issued new Family and Medical Leave Act (“FMLA”) notices and medical certification forms. The new forms have an expiration date of May 31, 2018. The only substantive change to the documents is the addition of language instructing health care providers not to provide information that would be covered by the Genetic Information Nondiscrimination Act (“GINA”). These notices and medical certification forms should be substituted for those that have an expiration date of March 1, 2015. A link to the new forms can be found here.
